For Wisconsin families in CLTS

More Independence. Less Worry.

BoundaryCare is a cellular safety watch, set up for your child, that alerts you if they leave a safe zone and lets you call them anytime. It may be available through your child’s CLTS plan — most often as a Personal Emergency Response System (PERS).

  • Works without wifi
  • You get the alerts

What families use it for

Room to Explore, with a Way to Reach Them

Safe Zones and Location

Set safe areas like home, school or a grandparent’s house. If your child leaves one, you get an alert right away — by app, text or call — and can see where they are on your phone.

Call and Be Called

Call the watch anytime — it can answer on its own so you can talk right away. Your child can call you with a tap.

Reminders and Routines

Gentle reminders and step-by-step prompts help your child do more of their day on their own.

Seizure Symptom Monitoring*

Get an alert when the watch picks up movement and heart-rate patterns that may be seizure symptoms, day or night. Available with select kits.

Depending on the kit, BoundaryCare also offers fall detection, heart-rate and sleep insights, and video calling.

* BoundaryCare is not a medical device and is not intended to diagnose, evaluate, or treat any condition. Learn more about seizure symptom monitoring.

How it works

Simple for Your Child. Clear for You.

  1. Your Child Wears the Watch

It arrives set up for them and works on its own cellular service — no phone needed nearby. Wear it on the wrist, in a secure band, or on a clip.

  2. You Choose Who Gets Alerts

Add parents, family and other caregivers. There’s no limit on care team accounts.

  3. You Respond Your Way

Check their location, call the watch, or send a message — from the BoundaryCare app on your phone.

You stay in charge. You and the caregivers you choose get the alerts. BoundaryCare staff don’t monitor your child’s location — our support team only looks if you ask us to, to help with a technical issue.

How the CLTS request works

Five Steps from Asking to Wearing It

You don’t need to know billing codes or fill out special forms. Your coordinator and your county make the decision; we handle the quote and the billing.

  1. Get the Request Kit

A short guide for you, a one-page summary for your coordinator, and a message you can send.

  2. Talk with Your Coordinator

Share the kit with your support and service coordinator (many families say “case manager”).

  3. We Prepare the Quote

Your coordinator contacts us. We send the quote and product information the plan needs.

  4. Your County Decides

BoundaryCare is most often authorized as PERS, sometimes as Assistive Technology.

Decision within 14 days of your request
  5. It Arrives Set Up

Once it’s in your child’s plan, we bill CLTS directly, ship the watch ready to go, and help you get started.

Every CLTS plan is decided for each child by the county, so we can’t promise approval — but we’ll work with your coordinator the whole way.

Questions families ask

Good Questions, Straight Answers

Is BoundaryCare automatically covered by CLTS?

No. Every service in a CLTS plan is decided for each child by your county. BoundaryCare is most often authorized as a Personal Emergency Response System (PERS), and sometimes as Assistive Technology. When it is authorized, we bill CLTS directly — there’s nothing for you to pay us for the authorized items.

Is BoundaryCare a CLTS provider?

Yes. BoundaryCare is listed in Wisconsin’s CLTS Provider Directory and serves all 72 counties. You or your coordinator can look us up at cltsproviderdirectory.wi.gov — search “BoundaryCare.”

What do I say to my coordinator?

The kit includes a message you can send by email or text. It asks your coordinator to consider BoundaryCare for your child’s safety and independence goals. You don’t need to know billing codes — the coordinator summary explains that part.

Does my child need to be in CLTS?

For CLTS to pay for BoundaryCare, yes. If your child isn’t enrolled yet, your county health and human services agency can tell you about the program, and the Wisconsin DHS CLTS page explains it. You can also buy BoundaryCare directly.

How long does it take?

Once you ask for a service, your county has 14 days to decide (it can extend once, up to 28 days). After BoundaryCare is in your child’s plan, we set up the watch and ship it to you.

Who gets the alerts?

You and the people you choose — parents, family members and other caregivers. Alerts come by app, text or phone call. BoundaryCare staff don’t monitor your child’s location; our support team only looks if you ask us to, to help with a technical issue.

What if my child won’t wear it?

Many kids do better with a different band, or with the watch on a clip. There are several options, and we’ll talk through them with you before anything is ordered.

My child is 18 to 21. Does this still apply?

Yes — CLTS supports young people until they turn 22. For young adults, BoundaryCare works best when they help decide how it’s used and agree to wear it.

Can I buy it directly instead?

Yes. You can order online, and we’ll set it up for you. Financial-aid pricing and a 30-day money-back guarantee are available for direct purchases.

What happens after I fill out the form?

Your kit opens right away, and we email you a copy. Someone from BoundaryCare may follow up to see if you’d like help. You can also reach us anytime at wisconsin-clts@boundarycare.com.
