Connect with your Care Manager & HES Provider

Your care manager can order an evaluation for Home-Enabling Supports (HES), a new service line within your OPWDD waiver that can fund technology and services that improve your independence in the community. The HES provider will formally evaluate BoundaryCare’s appropriateness.

Order and Setup

Once your assessment is completed, your care manager will work with the HES to add BoundaryCare to your plan and finalize the purchase. Our team is available to assist with setup and ongoing support as needed, ensuring a smooth transition to using BoundaryCare.
